Perinatal Outcome of Triplet Pregnancy

Abstract
Objective: The aim of this study was to evaluate the perinatal outcomes and maternal complications of triplet pregnancies.

Methods: This was a retrospective cohort study of triplet pregnancies in single center, Seoul National University Hospital in Korea between 1997 and 2011. We reviewed maternal and neonatal medical charts of triplet pregnancies which were continued more than 20 weeks¡¯ gestation. Delayed pregnancies were excluded. We evaluated gestational age at delivery, obstetric outcomes, postpartum complications, gestational age and cord pH according to the birth order, neonatal outcomes and congenital anomalies. Statistical analysis was performed using Student¡¯s t-test, Fisher¡¯s exact test, and ¥ö2 test, ANOVA with Bonferroni adjustment as appropriate.

Results: The total number of deliveries of triplet pregnancies increased from 2007 to 2011. Gestational age at birth was significantly increased from 31.6 ¡¾ 4 weeks¡¯ gestation during 1997-2001 to 34.1 ¡¾ 2.0 weeks¡¯ gestation during 2007-2011. Deliveries ¡Ã 34 weeks¡¯ gestation were 63.6%, deliveries ¡Ã 30 weeks¡¯ gestation were 87.5%, deliveries < 28 weeks¡¯ gestation were 5.7%. Average birth weight was 1.79 ¡¾ 0.51 kg (mean ¡¾ SD). The most common maternal complications during pregnancies were preterm labor and the most common postpartum complications were anemia. There was no hysterectomy or maternal mortality. The survival rate of neonates was 95.4% and the composite morbidity was 13.6%. Congenital anomalies were found in 4.2% of triplet neonates.

Conclusion: During the last 4 years pregnancy outcomes and perinatal outcomes of triplet pregnancies were improved. We report good outcomes on the basis of postpartum maternal complications and neonatal survival rate in triplet pregnancies.
